Tips on using Submittable:
- You will need to create a free Submittable account or sign in with Google or Facebook credentials to submit to these forms.
- You can save a draft of your work if you would like to finish filling out the form at a later date. There is an autosave feature, but you can also manually save your draft by clicking the button at the bottom of the form.
- Submittable works best on Google Chrome, Firefox, and Safari. Internet Explorer is not supported. Please make sure you are using a supported browser.
- We will follow up with you about your submission by email. Please be sure to safelist notification emails from Submittable and check the email you used to sign up for your Submittable Account regularly.

College Choice Program
Welcome to the College Choice Program application, an initiative from NYC’s Administration for Children’s Services (ACS), in partnership with The New York Foundling.
College Choice supports youth in foster care who attend accredited two-and four-year college full-time, regardless of location. The following benefits are available for eligible students. Please see the College Choice webpage for complete program details including eligibility requirements.
Important Tips to Consider Before Starting Your College Choice Application
- You may want to ask your Coach, Education Specialist, Case Planner or support staff at the foster care agency to help you work on your application
- Make sure you have a secure internet connection; using a desktop is preferred but you can also use your cellular device
- Submittable is best accessed using Google Chrome
- The application will take about 30 minutes to complete. You can save your work and return later if you need to (remember to save your log-in credentials)
- Make sure you secure the following contacts: (Email & Phone Numbers)

- Education Specialist
- Housing Specialist
- Fair Futures Coach OR LifeSet Specialist (if applicable)
- Gather the following documents:
- Enrollment Verification
- Bursar Bill
- Transcript (not applicable for first time freshmen)
- Course Schedule
- Academic Calendar
- For Post-Graduation Students, gather the following documents:
- Final Transcript (Should Indicate “Degree Awarded”)
- Diploma
